Registration Dos and Don’ts

Club Directors
DO:  Send me a Club Designation Form.  List all teams.
DO:  Include your email address.

Team Representatives
DO:  Send a Team Registration Form, along with Individual Registration Forms
for each team member (player, team representative, coach, and chaperone) in one envelope.
DO:  Include payment for the team fee and payment for each team member.
All payments should be made out to CHRVA.
DO:  Send a Team Member Add Form along with Individual Registration Forms
and payment for any new team members.
DO:  Include your email address.
DO:  Print legibly.
DON’T:  Fill in column for Registration Numbers.

Individual Team Members
DO:  Give all forms and payment to your Team Representative.
DO:  Sign pages one and two of the Registration Form.
DO:  Print legibly.
DON’T:  Send me an Individual Registration Form if you are a member of a team.

Individuals Not Associated with a Team (Officials, Club Directors, Board, and Staff)
DO:  Send me an Individual Registration Form.
DO:  Send payment made out to CHRVA.
DO:  Sign pages one and two.
DO:  Print legibly.

Everyone
DO:  Check the CHRVA website for Online Registration updates.  The system
will soon be made available to all, and we’ll make the link to it as visible as possible to the membership.  Please bear with us as we launch the new system.  There will be bumps along the way as we make improvements and fix glitches, but we hope to put in place a much more efficient way to register teams, players and other personnel.

As of Thanksgiving Day, there were 128 teams and 1320 members registered in the Region.
