The 2001 USA Nationals will be held in Milwaukee,
Wisconsin, May 26 through June 2. If your team would like to attend
this year’s Nationals and need entry booklets or additional information,
please contact the CHRVA Commissioner, Anne Hughes (anne.hughes@chrva.org
or 703-451-3082). The entry fees are as follows:
Open Tournaments: $850
Club, Masters and Seniors Tournaments: $750
Regular and Reverse Co-Ed Tournaments: $250
Entries must be received by Nationals not later than March 28, 2001
for
the Open tournaments and April 11, 2001 for the Club,
Masters, Seniors, and Co-Ed tournaments.
The CHRVA Commissioner MUST verify all entry forms. Therefore,
send the original completed form to the CHRVA Commissioner who will
provide a verification signature and forward the entry form to the USA
Volleyball National Office. The mailing address is:

Send a copy of your entry form directly to the National Office along
with payment. Make checks payable to USA Volleyball Championships

Please note that no team will be officially entered into the National
Tournament until the original entry form has been signed by the Region's
Commissioner and received by the National Office.
Send a completed (as much as possible) Hotel Reservation Form to the
National Office along with the entry information. The Hotel Form
