The 2004 USA Volleyball National Championships
will be held in Atlanta, Georga from May 29 through June 5.
If your team would like to attend this year’s Nationals and need entry
booklets or additional information, please contact the CHRVA Commissioner,

or 703-451-3082). The entry fees are as follows:

Note the following additional charges:

An additional $100 processing fee is charged when not using
TIVA, the On-Line Registration System.

An additional $100 fee for insurance is charged for all Non-USA
teams in the Open, Master and Senior
tournaments.
For 2004 Open tournaments, entries must
be received by Nationals
not later than 5:00 PM MDT, Wednesday, March
24, 2004.
For 2004 US Club, Co-Ed, Masters and Seniors
tournaments, the National Office will only accept entry forms forwarded
from the CHRVA Commissioner. The CHRVA Commissioner MUST verify
all
entry forms. The CHRVA Commissioner has established April 1, 2004
as the cutoff date for signing and checking all entries before she sends
them to the USA Volleyball National Office. Please see Page 28
of The
Pre-Tournament Manual for instructions on how to use the TIVA
Online Entry System to create and print your Official Entry Form which
includes your player and team information. In other words, use
TIVA to avoid paying an additional $100 processing fee. And, get
your paperwork and checks to the Commissioner by April 1, 2004.
Exception
Due to the required number
of teams to conduct any tournament, all Masters 30 and 35, Seniors 60
and above, U-Volley, and Co-Ed teams MUST get their paperwork
and checks into the Commissioner's hands by March 15, 2004.
This deadline was established in order to relieve players/teams of the
liability of potential financial loss due to plane and hotel reservations.
These teams should still use the TIVA system to avoid paying the additional
$100 processing fee.
Therefore, send the original completed
form to the CHRVA Commissioner who will provide a verification signature
and forward the entry form to the USA Volleyball National Office.

Please note that your team's entry is not considered
complete, and will not be processed until the following is met:

the established deadline.

Failure to follow these procedures will jeopardize
or delay the acceptance of your entry application. Please make your
check payable to: USA Volleyball Championships, staple it to a copy
of your entry form and mail to the Commissioner along with all the required
forms for the tournament you are entering. On the Memo line of your check
PRINT
the team name and division.
It is crucial for every Nationals candidate
team to download and print a copy of The Pre-Tournament Manual for
the 2004 USA Volleyball Championships. The document is available
for download at http://www.usavolleyball.org/media/adult/04_PTM.pdf.
The Pre-Tournament Manual contains a wealth of information regarding registration
requirements, costs, tournament format, rules, hotels, travel and much
more.
Teams should fill and submit the 2004 Housing
Form as soon as possible. The Housing Form does not need to be
sent to the Commissioiner.
