Registration with the Chesapeake Region Volleyball Association (CHRVA) consists of two parts; Individual and Team. 

Individual Registration can be completed using any of three (3) methods.

Method 1) Off-Line

Download "USAV Individual Registration Form" at http://www.chrva.org/Forms/USAVmembershipform.pdf.
Print the form and waiver, fill in appropriate fields, sign the form, sign the waiver (page 2).

If you have committed to playing with a specific team, then,

Send the Registration Form, Waiver, and Registration Fee (Check Payable to "CHRVA") to your Team Representative who will forward to the CHRVA Registrar with the Team Registration materials.

If you have not committed to playing with a specific team, then,

Send the Registration Form, Waiver, and Registration Fee (Check Payable to "CHRVA") to the CHRVA Registrar at;

Method 2) On-Line

Enter the CHRVA Online Registration System, at http://www.vball-life.org/register.

Note:  Send technical questions regarding the operation of the Online Registration System software to the Officials' Website Administrator at charles.ezigbo@chrva.org.

Follow directions to complete your Individual USAV Registration.
Pay Online with a credit card to immediately obtain a CHRVA registration number.
Print and Sign the Waiver Form.
If you have committed to playing with a specific team, then,

Send the Signed Waiver Form to your Team Representative who will forward to the CHRVA Registrar with the Team Registration materials.

If you have not committed to playing with a specific team, then,

Send the Signed Waiver Form to the CHRVA Registrar at;

Method 3) Last Resort

If, for some reason, are unable to either print the appropriate form(s) or use the OnLine Registration System, then please contact the Registrar (registrar@chrva.org) for help with the registration process.  If necessary, the Registrar can supply you with the appropriate forms.

Download the "Team Registration Instructions" from the CHRVA Forms Archive. Read them!
Download the "Team Registration Form" (either of two formats).

The Microsoft Word format allows information to be typed into pre-defined fields on the form prior to printing.
The Adobe PDF format requires information to be hand-written into fields on the form after printing.

Follow the previously downloaded "Team Registration Instructions".

Mail the signed Team Registration Form, the team registration fee ($40 check payable to CHRVA), the signed Individual Registration Forms, the signed Individual Registration Waivers, and the individual registration fees (check(s) payable to CHRVA) to the CHRVA Registrar at;
